Ze žádosti musí být zřejmé, kterému povinnému subjektu je určena, a že se žadatel domáhá poskytnutí informace ve smyslu tohoto zákona. Fyzická osoba uvede v žádosti jméno, příjmení, datum narození, adresu místa trvalého pobytu nebo, není-li přihlášena k trvalému pobytu, adresu bydliště a adresu pro doručování, liší-li se od adresy místa trvalého pobytu nebo bydliště. Právnická osoba uvede název, identifikační číslo osoby, adresu sídla a adresu pro doručování, liší-li se od adresy sídla. Adresou pro doručování se rozumí též elektronická adresa.
Pověřený pracovník je povinen prokazatelně informovat žadatele o prodloužení lhůty a důvodech prodloužení, a to ještě před uplynutím lhůty 15 dnů.

Information for whistleblowers of illegal acts according to Act No. 171/2023 Coll. Statement on the implementation of the EU Whistleblowing Directive.
In accordance with EU Directive No. 2019/1937 of 23/10/2019 on the protection of persons who report violations of Union law (hereafter referred to as EU Directive), with effect from 17/12/2021, and in accordance with Act No. 171/2023 Coll. ., on the protection of whistleblowers, is introduced by the organization 1st International School of Ostrava - international gymnázium, s.r.o. as a mandatory entity in accordance with Article 8 of the EU Directive the following ways and rules for reporting illegal acts.
The school, as a mandatory entity, has drawn up an internal guideline for this issue, which it publishes on its official board. The Directive establishes an internal notification system as a summary of procedures for receiving notifications, their registration and processing, protection of the identity of the notifier and protection of the information provided in the notification.
Any person - a whistleblower who becomes aware of a violation of Union rights in connection with work or another similar activity (EU Directive) can submit information about a possible illegal act that has occurred or is expected to occur:
either school or Ministry of Justice
The whistleblower will be informed about the rights and obligations arising from the law, and a record will be made of the information. The whistleblower should act in the public interest and in good faith that the report submitted by him is based on credible facts and circumstances. If the whistleblower knowingly files a false report, he commits an offense for which a fine of up to CZK 50,000 can be imposed. An audio recording or recording of the oral notification shall be made, if the notifier agrees.
Only the relevant person mentioned below may read the submitted notifications, who may not provide information that could frustrate or jeopardize the purpose of submitting the notification. The competent person shall assess the reasonableness of the submitted notification and inform the notifier of the receipt of the notification pursuant to § 12, paragraph 2 and of the results of the assessment of the reasonableness of the notification pursuant to § 12, paragraph 3, and of the adoption of appropriate measures to correct or prevent the illegal situation following the notification submitted. If the notification is not evaluated as justified, the competent person shall notify the notifier in writing without undue delay that, based on the facts stated in the notification and all circumstances known to him, he did not find suspicion of committing an illegal act, or did not find that the notification is based on false information, and instructs the whistleblower on the right to file a report with a public authority. If the notification is evaluated as justified, the relevant person will propose measures to the school to correct the illegal situation. The obliged entity shall immediately notify the relevant person of the measure taken, who shall notify the notifier in writing without undue delay.
The school excludes receiving notifications from persons who do not perform work for the school, or other similar activities according to the aforementioned law.
The identity of the whistleblower is protected information, the relevant person is bound by confidentiality. According to Act No. 171/2023 Coll., the whistleblower is entitled to protection against retaliatory measures. The Ministry of Justice will provide information on its website on how the whistleblower can proceed in the event of retaliation.

REPLACEMENT OF A FOREIGN LANGUAGE EXAM
COMMUNICATION OF THE SCHOOL IN ACCORDANCE WITH § 81 PARAGRAPH 7 ACT No. 561/2004 Coll., ON PRESCHOOL, PRIMARY, SECONDARY VOCATIONAL AND OTHER EDUCATION (SCHOOL ACT), AS AMENDED BY LATER TRANSCRIPTIONS
One of the four compulsory examinations in a foreign language or one of the two optional examinations in a foreign language can be replaced by a successful standardized test proving the pupil's language competence at level B2 or higher according to the Common European Framework of Reference for Languages and at level B1 or higher in the 2nd foreign language as an optional exam. In such a case, the pupil submits a written request to the school principal to replace the examination no later than March 31 for the Maturita examination in the spring examination period and by June 30 for the Maturita examination in the autumn examination period of the given school year (§ 19 and para. 177/2009 Coll., As amended). The application always includes a certified copy of the proof of successful completion of the standardized language test. At the same time, the student submits the original document (certificate) to the school principal for inspection.
